I provide a streamlined, three-step diagnostic evaluation conducted entirely from the comfort of your home via a secure, HIPAA-compliant telehealth platform.
I use a dual-method testing approach to ensure deep diagnostic accuracy. This phase combines two distinct types of evaluations:
- Self-Report Rating Scales: Standardized, evidence-based questionnaires sent securely to your patient portal. You will complete measures such as the Adult ADHD Self-Report Scale (ASRS) to assess current symptoms and the Wender Utah Rating Scale (WURS) to evaluate childhood symptoms.
- Objectively Administered Tests: Live, interactive clinical exercises completed during a video session. This may include Continuous Performance Test (CPT) to measure attention over time and a Brief Intelligence Scale to evaluate core cognitive strengths and executive processing.
Investment
- Total Cost: $750 (Flat fee)
- Includes: Intake interview, testing administration, scoring, formal diagnostic report, and the results consultation.
- Payment is due at the time of scheduling. I provide superbills upon request for potential out-of-network insurance reimbursement.
Virtual Bariatric Surgery Psychological Evaluations
I utilize a robust suite of standardized instruments to ensure a thorough assessment of your psychological readiness, coping mechanisms, and behavioral health. This comprehensive evaluation includes:
- Eating & Substance Use Screening: You will complete the Binge Eating Scale (BES) to assess emotional eating patterns and the Alcohol Use Disorders Identification Test (AUDIT) to review substance use safety.
- Mood Screening: Quick, standardized screeners including the Patient Health Questionnaire-9 (PHQ-9) for depression and the Generalized Anxiety Disorder-7 (GAD-7) for anxiety to evaluate your current emotional stability.
- Objective Personality & Medical Inventories: Comprehensive assessments using the Personality Assessment Inventory (PAI) to evaluate general clinical symptoms and interpersonal style, alongside the Millon Behavioral Medicine Diagnostic (MBMD) to specifically assess psychosocial factors that influence your adherence and coping with medical interventions.
I interpret your data and compile a formal psychological evaluation report. I handle the paperwork efficiently to avoid delaying your surgery date:
- Direct Delivery: I send the official clearance letter and comprehensive report directly to your surgical team.
- Turnaround: Reports are typically finalized and sent within 7 to 10 business days of our session.
